Check your graphics settings. If what you're seeing doesn't make sense for the graphics settings you're using, please file a
support ticket for a technical bug.

Two basic reasons:
1) On most ships with figureheads, it's integral to the ship model and textures themselves. And many of these are done by community members. The only way we could realistically achieve the goal of adding figurehead variations in this way is by getting each and every shipwright to redo their work for however many figureheads we have.
2) Even with just two figurehead variants, this quickly becomes more ship models -- and more ship recipes -- than is really healthy for the game. E.g. right now if there are two Postillions in an instance, your computer basically just needs enough resources to draw one (that's a bit oversimplified for explanatory purposes). If one of them is the "mermaid" variant and the other is the "unicorn" variant, though, now your computer needs to draw them separately. Never mind the fact that by offering two variants, you really and truly beg for having 5 or 10.
In other words, the best way to customise a ship itself is through the paint job and the sails, which you already can do.
